OSHA Injury and Illness Reporting Requirements
As reported by OSHA, 4,679 American workers died on the job in 2014.
OSHA injury and illness reporting regulations require employers to report fatalities as a result of a work-related incident within 8 hours, and in-patient hospitalizations, amputations, or loss of an eye as a result of a work-related incident within 24 hours. For your convenience, reporting options follow:
- Call OSHA’s free and confidential 24-hour hotline at 1-800-321-OSHA (6742).

- Use the new online form, which recently became available as a method of reporting. The “Serious Event Reporting Online Form” can be found at https://www.osha.gov/report.html
OSHA’s On-site Consultation Program offers free and confidential safety and occupational health advice to small and medium-sized businesses in all states across the country before accidents occur, with priority given to high-hazard worksites. On-site Consultation services are separate from enforcement and do not result in penalties or citations.
